  I get an empty page when I try to look at the T & C's via the link on the form irrespective of whether I use IE8, Firefox or Goggle Chrome. Anyone else in same boat? Date and time is 19:12 - 7th May 2012.

  I use IE9 and did not have any problems. Did you click the 'certainly have' field?
  It's a bit ironic because the T&c's are missing but you need to check that box to get a submission.
