With all those fees/subscriptions you've rightly highlighted, its probably better off using VoIP alternatives such as the Betamax brands such as JustVoIP, InternetCalls, VoipBuster. You have to sort out your own hardware (ie handset/headset/phone/whatever) and they take an initial upfront prepayment of 10 Euros/pounds off you plus TAX but then typically let you make free calls for 120 days (subject to 300 minutes per 7 day period) and even beyond that only 1c/minute or so to use up your 10 credit (which doesnt expire). Many of the a/c's support SIP. The differing brands vary as to which countries are 'free' and paid country costs (even in UK) can vary a lot so choose the brand that suits your profile of calls - eg JustVoip is quite good for mostly UK calls. You don't get much/any customer service but I and friends on the Continent have found the service has been very stable/reliable for the last 18m or so.
This site summarises the brands/costs (in Oct 07 anyway) http://backsla.sh/betamax
