Since it provides short cuts to those settings it won't work if you disallow them. The app isn't attractive to me but for those who have a use for it, understand that if you are using an app that provides shortcuts to "Contacts" for example, it will need to have access to your contacts list in order to set a shortcut to it. A shortcut to your picture gallery will require access to your sd card, USB storage and media files. Same applies for the other requests. I guess it comes down to how much trust you can place in the developers of the app because the potential for abuse of those permissions is there. If in doubt don't install. If you do install check on its behaviour every now and then just as a precaution.

a lot of these apps go under the name "floating toolbox" and cause a lot of apps to stop working for security reasons and give an error about a screen overlay.
