  I don't know if this is working or not.
  I answered the questions and filled in my address. Clicked and was taken straight back to the address form. I tried again and the same thing happened.

  Takes you back to the address form but if you scroll down it says your sample will be with you within 14 working days. Thanks OP!! :)

  Silly Me!! I paid more attention and saw that all the boxes are red *, so I put a number in for the house number, that I don't have!

  Tried this as well now and it works. My address is not long enough to get address 2 field populated so it did not go through.
  
  I put address 2 field to be the same as Town / City and it went through with confirmation message.

  BTW if you say NO to do you have sensitive teeth it will tell you that you are not eligible to receive a sample.
