I have a web site hosted by 1&1. Do you mean response forms that will be emailed back to you automatically?
I couldn't get this to work, but used Responsomatic instead. You can build any form you want, then paste the HTML code into your page.

Not sure about your question.. but with your 1and1 domain... do they display all your address and telephone number on the whois? i registered some domains and it shows EVERYTHING :(

5nowman when you register the domain name you need to check a box to hide your details on a who.is

disco, you should be able to do that, you just need to use the code into your site, send me an email thru msn
