if you over 18+ and unemployed (not seeking JSA), how much is it usually to visit the dentist.

NHS dental charges

The three NHS charge bands are as follows:
Band 1: £16.50. This charge includes an examination, diagnosis and preventive advice. If necessary, it also includes X-rays, scale and polish and planning for further treatment.
Band 2: £45.60. This charge includes all the necessary treatment covered by the £16.50 charge, plus additional treatment, such as fillings, root canal treatment or extractions.
Band 3: £198. This charge includes all the necessary treatment that is covered by the £16.50 and £45.60 charges, plus more complex procedures, such as crowns, dentures and bridges.

BG if you dont have a regular income/no income you can apply for a card (I believe it's either a HC 2 or HC 1) which either exempts you from dentist fees or gives you reduced payments depending on a low income set rate.

Are you registered with an nhs dentist?

If not find out the nearest nhs dentist to you who is taking on new patients

If you have no money or on benefits then most of the time its all free...your dentist can advise all of this

Dont think theres a form online mate, I had to search it on the NHS site and they sent me one first class. Although I found out I didnt need it cause I'm still full time edu.

you can't. Some are available from chemists though, depending which you need. The information is in the links i posted above.
