You need it connected up and then click detect displays or go to display options and select "detect displays whenever CCC starts <

You could try connecting just the HDMI and restarting your PC,

If I am reading right, the issue might be that CCC is seeing the hdmi/dvi connection as a 2nd monitor so you need to click extend display or make primary to get it to view on that screen.

Connect it up, turn it all off, then turn TV on, set it to the right input, then turn the PC on.

I had this issue with my monitor, with the catalyst drivers installed the DVI to HDMI cable didn't work. As I was getting an HDMI to HDMI cable and that worked with the included DVI to HDMI adapter I just put it down to a poor cable.
