Hi All,

As I'm getting married I was looking at getting my name changed before to include a middle name that I have been known by for years, but it's never been an official name!

I have seen this service http://freedeedpoll.co.uk/free-deed-poll.cfm but just wondered if anyone had used them?

I used my own set of documents, all you need to do is edit the relevant details print it out and get it signed by someone else as witness, all legally accepted. PM me your email address and I will email you them in word and pdf formats if interested. Cant remember where I got them from but they are legit and legal.

[SIZE=3][COLOR=black][FONT="]Original Deed Poll provides two easy ways of making the Deed Poll document. On page three text is given in red and black fonts. Red text shows you how to correctly complete the blank Deed Poll document on page four. Alternatively you can delete the Red Italic and fill in your new and old names accordingly. You shouldn’t use the first page of this document which says ‘Original Deed Poll’ and also this page called ‘how to complete your deed poll’ when submitting the deed poll as evidence of name change.[/FONT][/COLOR][/SIZE][SIZE=3]

[COLOR=black][FONT="]Signatures[/FONT][/COLOR][/SIZE] [SIZE=3][COLOR=black][FONT="]Three signatures are required to complete the certificate. The person changing his/her name must sign both old and new signatures in the spaces marked (One can retain the same signatures even). The person witnessing the name change must also sign the form in the space provided. All three signatures must be completed in Black ink.[/FONT][/COLOR][/SIZE][SIZE=3]

The witness is signing that he/she has witnessed the person who is changing name sign both old and new signature onto the deed poll certificate. [/FONT][/COLOR][/SIZE] [SIZE=3][COLOR=black][FONT="]Once Deed Poll is completed it can be sent to the Passport Office, DVLA, and all companies and organisations, which hold your personal details, (Banks, Electricity/Gas/Water supplier, employer etc.); to provide evidence of your name change so that stored records can be changed to reflect the name change.[/FONT][/COLOR][/SIZE]
