How about a savings account for them. You could start it off

What are you looking to spend in total?

how about them personalised gifts you can get on ebay, somehting they can keep forever, get their name and dob printed on a trinket box/plate/silver dummy etc, or you can buy little silver bangles or gold ones from argos with nursery rhymes engraved in them,

For my friends first child, (who again had everything bought already) i made a hamper of things for my friend! Bubble bath, pampering things, mini perfumes, some of her fave choccies etc etc (and a couple of body suits for the baby!) The baby had everything therefore i thought i'd spoil my friend instead.

Theres a website called Notonthehighstreet that does beautiful silver charms with personalised finger prints but you would have to wait util the babies are born.

how about making a booklet of vouchers of things you can do for her after having the twins without feeling awkward asking for help, ie "I will do some ironing", "I will babysit", "I will do the washing up". Whatever you think she might need help with and you wont mind doing I done this for a friend and she was thrilled.
