You need to access the Microsoft Rewards app on your Xbox one console in order for the punch cards to be activated.
Once you launch the app go to the earn tab and all of the punch cards are available and simply scroll down the page for more.
For EA access both trials for UFC and Madden are now available via the app and the store.
All of the rewards do work as long as you have activated the punchcard.
