The process is very simple, me and many others have bought digital PS Store things from Amazon with no problem.
US Plus cannot be used on a UK account to get free games/discounts from the UK region but you can get the free games from the US Store aswell as play online multiplayer on your UK account by having Plus on another on the same console.
First if you already have a UK Amazon account they'll prompt you to select a US address. Can be anything but make it a Delaware address because they have no taxes on digital sales. (Naughty I know but with Amazon,Apple et al doing the same who can argue against doing it?)
You can then use your UK debit card to checkout as normal and the code will be sent via email as soon as it goes through. Because it is a US transaction Amazon US do state that they can hold the transaction for a few hours to make sure its not fraud. Normally it takes roughly 4 hours for everything to process and I've never had a problem.
Then redeem the code on your US PSN account (If you don't have one it take about 5 minutes to make, you just need to use a US address). After that you're good to go.
This process is not limited to just Plus. Amazon US always have deals on full PS games ( I bought digital AC4 for £25 on PS4 this way) so anything that states it is a digital code you can buy and redeem on a US PSN account on your console. Once its redeemed and downloaded you can play the game on any account on that console. So you won't have to start a new trophy list.

afaik...US / R1 psn+ free games as good as UK/R2 games

Never knew I could get PS Plus on my US account and it'd would give my UK account Plus too.
Sometimes they are, but certainly not always. Take this month for example:
In North America, the June lineup is PixelJunk Shooter Ultimate (PS4), Trine 2: Complete Story (PS4), NBA 2K14 (PS3), Sly Cooper: Thieves in Time (PS3 / PS Vita), Terraria (PS Vita), and Mutant Mudds Deluxe (PS Vita / PS3).

1. Is it best to set up a completely new USA account on Amazon or just have an address stored in the USA?
2. Can you create a USA Psn online without problems if location?

Tbh mate, its a bit of faffing about, but you still need to keep both ps+'s topped up if you want to download uk ps+ stuff. Also once your uk ps+ sub runs out, the games will be locked then.
So you saying its better to spend twice on sub's?

1/ I just added a US address when they asked me to. If you want everything to be separate you can easily make a Completely new account.
2/ Yes it's extremely easy to make a US account. Again all you need is a US address and you're good to go. Same with any region like Jap. It's a very common practise to have multiple region accounts to get access to demos/games not available on your home regions store.
3/ Yep any account on that console can play that game and even earn separate trophies with no issues.
4/ Never. This is not illegal or even immoral. Sony allow accounts to enjoy the benefits of another having ps plus. All you are doing is buying a subscription for your US account. Just so happens you're not in the US

Lol I know what you mean. Might be worth doing to get a cheap game in the future, but for ps plus I would just stick to a uk one for 8 quid more.
Possibly but I'm not sure what the benefits are of having to maintain two subscriptions (twice the price) and is it worth paying double for that little bit extra.
On ps3? No, that's free. On ps4? Yes, but the game isnt out on ps4 yet.
Yes you can. If one account has Plus on your console you can play online multiplayer on any account on that console.

No Sony made a concious decision to allow Plus benefits to be shared across accounts on a console. Making people buy a separate subscription just to play online on a different account would've started a war.
