Measure your feet in cm, it's the best way with cycling shoes
I found out that Specialized used to be 1 size bigger so a 12/47 will need a size 46 in a Spesh shoe.
It depends on the manufacturer. Shimano - you need one size up so a 12/47 will need a Shimano 48.

It's not just about measuring your feet, you also have to find out what the manufacturer do with their sizes too

Hope that clears any confusion I made earlier. Usually you ask other users who have bought the shoe and I don't have a LBS that I can try - I'll end up buying it at their price to support them lol.
