I tried this but after filling in the form at the beginning asking how many TV's I own, Age etc I was not eligible to do the survey and no reason was given.

Suggest you always hover your cursor over the e-mail address to check where the 'offer' came from.
I received an e-mail offer, ''would I like to complete a survey 'for Costa' for the chance to be entered into a draw for a £100 Costa reward card''.
The source e-mail address was... [email protected]chetergeor.com , which leads back to a very recently registered web address, which in turn seems to link further back to a UPS store (post office in effect), in 6311 Van Nuys Blvd , Van Nuys, CA 91401, United States; also to webhosting 'Pencevariety.com' in Dallas Texas. Costa Coffee is a UK company based in Dunstable, Bedfordshire, owned by Whitbread, also headquarterd in the UK.
From Costa... hmmm...not safe to assume so, no mention of 'Costa' in the email address either! If it looks like spam.... Delete!
