Go to the Windows 10 store, click your account picture and select redeem code. Just copy and paste the code and it redeems to your MS account which grants you Xbox AND PC access.

I redeemed the code online and it appeared both in the Windows Store and on my XBox

No, it most certainly won't. Latest example is Dead Rising 4. Happened the same. Steam version is different from Windows Store and not transferable.

Sadly that's steams fault to some degree. They've yet to figure out how to allow people using microsofts network (remember this is cross play, so needs to connect to Xbox Live servers) or Valve are being awkward. The only cross platform title from memory was Portal 2, and that was only between PSN and Steam. 343i apparently are looking to try and get the cross play stuff working - though even then I imagine it'll just be PC to PC rather than PC to Xbox.

Dead Rising 4 is a horrid example. That's not even a cross buy title on the Windows Store (separate game lists for windows 10 and Xbox One). God knows what idiot thought that would go down well.
