    exemption certificate for free dental treatment

    hi i am registering at a dentist today and on the website it says by law they need proof of why you are entitled to free treatment, i have a 4 month old baby and i believe you get free treatment till the baby is one. how do i prove this as i havent got an exemption certificate, if i took my babies little red book do you think that would be ok.

    hi there so you dont have your card then really they need to see this as it has a number on it ....cant you see if they can send you another...let me see if my card has a number and yes you DO get free dental work

    To apply for your maternity exemption certificate ask your doctor, nurse, midwife or health visitor for form FW8. You are required to complete parts 1 and 2 of the form and your doctor, midwife or health visitor is asked to sign to confirm the information given by you is correct.
    Your MatEx will last until 12 months after the expected date of birth. If your baby is born early, you can continue to use your exemption certificate until it expires. If your baby is born late, you can apply for an extension. If you apply after your baby is born, your exemption certificate will run for 12 months from your baby’s birth.

    NHS prescription charge certificate - maternity exemption cert no....
    valid from and expire end
    the surgery will need the cert no and you will need to complete a form and sign it when you have any treatment free:thumbsup:

    oh i see your midwife should of filled a form in for you when you go for your first check up with her at 12weeks strange that defo get it sorted its so worth it....takes a week to come thru I make sure I use it on my third crown with my second pregnancy:p

    i think as i didnt find out i was pregnant till i was 4months gone, everything was a rush to get me in for a scan etc i hate dentists but ive had tootchache for past couple of weeks so want to get it sorted.

    got my card through quite quick - plus you get a year of prescriptions and stuff free after baby is born.
