    need a bit of advise on accessing medical records from hospital


    i am next of kin of a patient who was operated in 2008 in neurosurgery hospital, central london.now he is well but not indepenent as yet so lives in

    nursing home. he is going abroad and wishes to have all his medical records with him, so in case of any emmergency medics abroad can refer to the past

    history.its 2andhalf year old case now.


    Need your advice about this, how to access and get a copy of his medical records from the hospital? who should i speak to in hospital?.

    your ideas and quick response is highly appreciated. many thanx in advance.

    Details of any treatment should be held in his medical records, which are held at his doctor's surgery. Ring the reception & start from there....

    When you are discharged from hospital you get a discharge sheet with a basic summary of what has taken place. The hospital should also send a copy to the GP.
    I wouldnt think it was possible to get a full medical history but maybe he should take copies of any discharge sheets and any letters for outpatient appointments he may have.

    You will need to speak to the GP anyway to ensure he has access to adequate medicines he may be taking.

    Phone the hospital where he was treated - ask to speak to medical records, explain what you want and they will be able to explain if they can help you and what the procedure is.....

    thank you every one for the words of advice. appreciate your quick responses. i just spoke to the hospital medical records section and they do provide medical records. they will send me an application form which i have to submit with a fee of GBP 50.

    This was first time i am accessing med. records and great learning for me. thanx every one.
