I tweeted Waitrose and they've provided me with a list of stores (nothing about this online not even confirmation they're doing it again!):

What is the minimum spend?

There isn’t a minimum for the free coffee, but I think they have added scanners to the coffee machines so you need to scan your MyWaitrose card before the machine will work. Can only get one coffee a day and need to bring your own cup

My local has started this back up so I had an email saying it was starting back up again.

The email states that it's a trial that runs until the 29th May

Reading on the list, I was there at the weekend but no sign of anything happening yet.
