I vaguely recall it came in early in 2019 (about 6-9 months after topups became mandatory for Priority, and around 6-9 months before Classic was removed from o2 for new customers). There was a thread about it on here, but as searching HUKD is such a random process I haven't been able to find it. It also seems the application of the rule is wildly inconsistent. I always assumed that any new activations after the date of the policy change on topups would follow the new rules, but that doesn't seem to be the case. It may be related to a specific batch of SIM cards?

O2 don't know their own T&Cs for the Classic PAYG tariff.
There are several different versions with conflicting terms on their website.
There's a whole O2 community discussion about it here with links to all the different places where O2 show conflicting T&Cs....
    community.o2.co.uk/t5/…284

The main drift is that whatever you read on their website is a mess, and it appears to be wrong.
Trusted O2 employees have confirmed the following which seems to match the reality that I've experienced with mine..
"make a chargeable call or text at least once every 6 months and a top up every 999 days"

I bought a Classic PAYG sim in 2018 and have topped up twice with £10.
Both my sons bought one in late 2020, they topped up once for £10, they have made 1 call every 6 months since.
All our sims are still active and the credit is still there.

If you want to be reconnected you'll be given a new mobile number.when was that change made?

That change is from one of the various conflicting T&Cs on the O2 website as mentioned in my previous comment.
I don't think it's right. A chargeable call every 6 months should be enough to keep it active.

    That's the one you need.
Top up once for £10, then set a reminder to make a call every 6 months to keep it active.
O2 don't make them any more, but some forward thinking traders bought a load before they were discontinued in 2018, and are still selling them on eBay. (edited)

    Screenshots will work all day.
The 25min timer in the app is a clunky attempt by O2 app designers to stop people grabbing the codes at 7am and not using them.
I always take a screenshot if I'm planning to use it later. Saves a wasted trip. (edited)

When did you first top it up? If it was before the change in Classic (early 2019? There's a thread on here about it but I can't be bothered searching) then you'll still be on the old terms (where a text every 6 months is enough).
