    That's the "holster top". A tin of paint can go in the middle and pliers/hammers etc go in the other slots. Moving the ladder without them all falling out? I suppose it does discourage anyone from actually standing on the top step.

    I have to admit on my ancient 6 step ladder I still have to stand on the top

    Murphys law states howver tall the stepladder you will find  a situation where you would want to stand on the top step.
